How to register

We have an open list and welcome requests for registration from patients living in or moving to the practice area.

The process of registration does take 10-15 minutes or so whilst the receptionist checks through your forms. We will only register you once we have a fully completed form – to save time answer all the questions. If you are wanting to register a child, you must also bring along the red book. We have a strict policy that you come between 14:00 pm – 18:30 pm Wednesday & Friday but on Monday & Tuesdays their are extended times  (14:00 pm – 19:30 pm) in which you can bring them back.

If you have a current NHS medical card please bring it in. If you do not have a medical card or cannot find it please complete a GMS1 form which can be downloaded from here. GMS1 Form

You will also need to complete our Lime Tree Surgery new patient registration form available from reception. Alternatively you can download the registration form from here. Please be aware that both child & adult registration forms need to have the checklist on the front page signed and dated.

If you have had a previous GP, with your consent we can obtain a copy of your medical records from them which will aid the registration process. Please complete this form for you or your child/ren and bring it along to the surgery with your registration form. PATIENT CONSENT TO OBTAIN MEDICAL RECORDS FROM PREVIOUS PRACTICE

We will make a note on your records if you prefer to see a particular doctor or nurse.

We recommend that new patients undertake a health check with a practice nurse within the first three months.
