A Summary Care Record is an electronic record which contains information about the medication you take, any allergies you may suffer from and any reactions to medicines you have had before.
Having this information stored in one place makes it easier for healthcare staff to treat you in an emergency.
You can choose to opt out by completing a Summary Care Records Opt Out Form which you can get from reception.
