Online registration
Due to Covid-19 we have made all registrations electronic. Please complete our online form and send the required evidence to nelondonicb.LTSregistrations@nhs.net. We will need the required information to process your registration application.
For Adults please send a Photo ID and a Utility Bill/Council Tax/Bank Statement dated within the last 3 months.
For Children under the age of 5 please send the first page of the red book. Over the age of 5 please send Photo ID.
